Add/Update Your Parental Information to the LMYA Database

If you feel your family information (or email address) is not in the LMYA Database, then please fill out the following form. Only enter parent information, not Player information.

The information entered will then be verified against the database by a LMYA Official. The information will then be added/updated to the database and your username and password will be emailed to you.

Please allow a minimum of 1-3 days for processing ...

Note: Once you receive a username and password, you will be able to login and add/modify your family information for both parents and players.
